The practice operates the Year of Care (YOC) model; this is a patient-centred approach between the practice and yourself to manage your long-term condition(s). If you have a long-term condition, you can expect to be invited for a YOC 1 annual birthday review where you will see a Health Care Assistant or Nurses (asthma and COPD only) for data gathering where they will discuss lifestyle habits, take bloods, and blood pressure. Your results will be reviewed by the GP and, if out of target, we will invite you for a YOC 2 appointment to discuss. If they are normal/in range, they will be filed into your notes and no contact will be made. Approximately two weeks later you can expect a results letter to be sent to you.
Attached Staff
We offer 250 appointments across our employed team. Fenham Hall Medical Group is part of the Inner West Newcastle Primary Care Network (IWNPCN). The IWNPCN is essentially a group of five neighbouring practices who have come together in an attempt to deliver exceptional patient care. Our PCN is made up of Fenham Hall Medical Group, Prospect Medical Practice, Elswick Family Practice, Dilston Medical and Betts Avenue. PCNs benefit from the ability to provide some services by sharing members of staff; this means that someone may work in a different practice on different days.
The PCN services include:
